Wellbeing Support Worker at Headway Sussex

Provide high quality, person-centred rehabilitation and reablement support to clients with an acquired brain injury at community venues, in client’s own homes and in other community settings. Promote choice and control, personal wellbeing, independence, improved quality of life, social interaction, inclusion and peer support.

Liaise effectively with clients, family carers, Headway teams and other practitioners in helping clients progress to meet support plan goals for their independence. Take on the role of individual client link worker to enhance quality of engagement and personal support.

Provide client support one to one and in group and workshop activities both in person and online. Work effectively with Headway volunteers within a combined staff and volunteer team and jointly with other practitioners and partner colleagues furthering the charity’s aims and values.

Facilitate and lead wellbeing support and reablement aspects of Living Well Programme services. Contribute routinely to Headway’s digital and social media programme, encouraging client and carer involvement and feedback.
