Take Home and Settle/Relative Support Coordinator at Age UK West Sussex, Brighton and Hove

We are recruiting caring, reliable, empathetic people to help older people return home after a stay in hospital. The Take Home & Settle team help escort older people home from St Richards Hospital in Chichester. You will be required to collect them from the hospital discharge lounge and settle them at home, making sure they are safe and comfortable, checking heating and other facilities are working. Ensure they have eaten and possibly do some light shopping as well as notify their network of support they are home.
The successful applicant will need a practical approach, good people skills and will require a full driving license and use of a car for taking patients home in.
