Arch Health CIC is a social enterprise delivering primary care to homeless and vulnerably housed patients.
East and Central Brighton PCN consists of 9 GP surgeries generally covering the Whitehawk, Queens Park, Kemptown, Hanover, Lewes and London Road areas of Brighton.
Patient Engagement Workers will be responsible for the project management of a distinct set of initiatives designed to early identify possible clinical conditions and risks, to improve the health and wellbeing, and hence the life chances, of East and Central Brighton residents. This incorporates a number of different public health priorities: vaccination; early diagnosis of bowel, breast and cervical cancer; chronic obstructive pulmonary disease; diabetes; health checks.
Our ideal candidates will be organised and able to work with diverse involved groups – clinical staff, patients, community groups – in order to deliver the health improvement outcomes set out by the PCN directors. They will also have excellent written and verbal communication skills with the ability to analyse and convey complex information with clarity, as appropriate for the audience. They will be comfortable working with people from different backgrounds and in varied situations and contexts.
